He then shifted focus to his efforts in reforming the U.S. healthcare system through his company, Cost Plus Drugs.
Billionaire entrepreneur Mark Cuban recently engaged in a conversation on Bluesky after a user suggested he could literally end world hunger without affecting his financial standing. Rather than dismissing the comment, Cuban responded enthusiastically, saying, "I'm in! Tell me how."
When another participant questioned Cuban's intelligence for not knowing how to use his fortune to tackle global hunger, Cuban responded with humor, admitting, "I'm dumb. I can't figure out how to end world hunger." He quickly redirected the conversation to a challenge he feels more capable of addressing: reforming the American healthcare system.
"I will continue working to transform healthcare in this country, making it more affordable and potentially moving towards Universal Healthcare," Cuban stated.
Concrete Actions Already Underway
Through his company, Cost Plus Drugs, Cuban has been actively involved in reducing medication costs by eliminating intermediaries that traditionally inflate prices. The company operates with a transparent pricing model, applying only a 15% markup on medications and providing a full online price list. To date, the list includes more than 6,000 medications. Cuban has been vocal about his criticism of pharmacy benefit managers, claiming they have shifted from cost-lowering entities to profit-driven companies. He elaborated on this by saying these managers have acquired insurers and retail pharmacies, and now they are expanding into drug manufacturing. "They got greedy," Cuban said in an interview with Fortune earlier this year.
The Complexity of Universal Healthcare
While Cuban is a firm believer that healthcare is a right, he acknowledges that implementing universal healthcare in the U.S. is not a straightforward task. In a post on X (formerly known as Twitter) in September, Cuban explained that countries with universal healthcare systems established them before the advent of modern, expensive medical technologies. The real challenge, Cuban pointed out, is how to deliver fair healthcare without financially burdening hospitals or doctors.
"The only challenge is how we design the system that works in this country," Cuban wrote.
An Alternative Approach
Cuban has proposed a model that would reduce the dominance of insurers by removing premiums and expanding cash-pay options with transparent pricing. This approach would give patients the freedom to choose care that fits their budget while allowing taxpayers to cover the remainder of the costs. Cuban added that this idea is still evolving, stating, "Still have work to do on the numbers, and this is off the top of my head." Under this model, patients would have the option to choose any cash-pay provider that honors the posted prices, a strategy Cuban believes could increase transparency and affordability in the healthcare system.
His approach is not focused on a single, sweeping reform but aims to make realistic, incremental improvements to the current system.

By emphasizing transparency, fair pricing, and gradual changes, Cuban hopes to make healthcare more accessible and manageable for Americans. His efforts are directed towards practical solutions that could be implemented within the existing healthcare infrastructure, with the goal of creating a system that works for all Americans.
