"The Pitt" was initially structured to consist of 12 episodes, each representing one hour in a standard 12-hour emergency room shift. On The Nocturnists podcast, creator R. Scott Gemmill noted that this 12-episode plan "made perfect sense."

However, when HBO Max acquired the series, they requested an expansion to 15 episodes per season. Casey Bloys, chairman and CEO of HBO and HBO Max content, explained the reasoning in an interview with Vulture. "A 12-episode season felt more like a streaming extension," he said. "HBO seasons used to run 12 episodes, then 10, and more recently eight. We wanted to push the boundaries. While 22 episodes wasnt feasible, 15 offered enough space for John Wells and the team to deliver something creatively satisfying."

Extending the season beyond the typical streaming series length increased the workload for the cast and crew, but it also brought advantages for both HBO Max and viewers. Bloys further clarified the commercial perspective behind this decision. "We aimed for 15 episodes to maintain 15 weeks of engagement," he said. "We also needed a budget that was sustainable." Unlike shows such as House of the Dragon, which can cost over $20 million per episode, "The Pitt" operates on a $45 million per episode budget.

Bloys also emphasized the benefit of shorter gaps between seasons. "This approach allows 'The Pitt' to return within a year, unlike series like House of the Dragon, The Last of Us, or White Lotus, which can take two years to produce. Getting 15 episodes in a year is a model Id like to see for more shows."

The second season of "The Pitt" is set to premiere in January 2026.
