What is Apple Cider Vinegar?
ACV is produced from fermented apple juice. Yeast first converts apple sugars into alcohol, which bacteria then transform into acetic acid, the main active ingredient in vinegar, explains Burroughs. The final product is mostly water with a small amount of acetic acid, containing trace minerals such as potassium, magnesium, phosphorus, and calcium, along with minimal polyphenols, which are insufficient for a significant antioxidant effect.
Unfiltered ACV may contain a cloudy substance known as the mother, which includes proteins, beneficial bacteria, and enzymes, according to Simmons.
Potential Health Effects
Research into ACVs health benefits is ongoing. A review in Frontiers in Nutrition found ACV helped reduce fasting blood sugar and HbA1c levels in people with type 2 diabetes. Another study in Current Medicinal Chemistry reported improvements in chronic disease risk factors, including lower blood sugar and cholesterol in certain groups.
Acetic acid may slow carbohydrate digestion, causing a slower rise in blood glucose, Burroughs notes. A meta-analysis in Nutrients also observed that daily ACV intake may promote weight loss and reduce waist circumference, particularly in individuals with overweight or type 2 diabetes, likely by increasing feelings of fullness and reducing calorie intake.
Preliminary findings suggest ACV might slightly improve blood lipid markers like cholesterol and triglycerides, though evidence remains inconsistent and further studies are necessary.

Simmons points out that ACV can slow digestion, potentially worsening gastroparesis, a condition common in people with diabetes that slows stomach emptying. Burroughs adds that while unfiltered ACV contains some bacteria, the amounts are small and not well-characterized, so it shouldnt replace probiotics or scientifically validated supplements.

Simmons advises limiting ACV intake to two tablespoons or less per day. Diluting it with water can reduce its acidity, and incorporating it into foods such as salad dressings, marinades, or pickles is often the most practical approach. Both experts emphasize that ACV should not replace medical treatment, medications, or lifestyle changes, particularly for individuals with type 2 diabetes or other metabolic conditions.
