The World Health Organization has unveiled new guidelines to accelerate the development of antibiotics targeting high priority bacterial infections. The initiative focuses on multidrug resistant infections and bacterial meningitis, aiming to guide research and enhance global health preparedness.
The World Health Organization (WHO) has issued comprehensive guidelines aimed at advancing the development of urgently needed antibiotics. The guidelines introduce specific Target Product Profiles (TPPs) to provide a structured framework for researchers and pharmaceutical companies working on antibacterial treatments. The initiative focuses on three major infections that are considered global health priorities.
New Target Product Profiles for Priority Infections
The WHO's newly defined TPPs target infections listed as high-priority bacterial pathogens. The profiles are intended to:
- Offer precise guidance for the creation of new antibiotics
- Promote collaboration between public and private sectors
- Minimize risks associated with research and development of treatments for high-burden infections
The three priority infection categories include:
- Severe multidrug-resistant (MDR) gram-negative infections
- Severe gram-positive infections in immunosuppressed and critically ill patients
- Bacterial meningitis
Addressing these infections is crucial for controlling severe bloodstream infections, pneumonia, urinary tract infections, and central nervous system inflammations among vulnerable populations worldwide. Bacterial meningitis remains highly fatal, with about one in six patients succumbing to the disease.
Current State of Antibiotic Development
According to WHO's Antibacterial 2025 report, there are approximately 90 new antibacterial agents currently in preclinical or clinical development stages. However, only a small fraction of these target the highest-priority pathogens, and even fewer are considered truly innovative. Dr. Yvan Hutin, WHO Director of Antimicrobial Resistance, emphasized that while recent development and approval of antibiotics is a positive trend, it remains insufficient to keep pace with the growing threat of drug-resistant bacteria. He highlighted the need for a consistent pipeline of innovative, affordable, and accessible antibacterial agents.
Understanding the Burden of Antimicrobial Resistance (AMR)
Antimicrobial resistance occurs when bacteria, viruses, fungi, or parasites evolve to resist available treatments. For bacterial infections, this means standard antibiotics may become ineffective. In 2023, about one in six laboratory-confirmed bacterial infections globally involved antibiotic-resistant bacteria. Earlier data from 2019 estimated that bacterial AMR directly caused 1.27 million deaths worldwide. Without significant intervention, AMR could lead to as many as 10 million deaths per year by 2050.
Global Efforts to Combat AMR
In October 2025, WHO urged member states, donors, and international partners to enhance coordination in combating AMR. The organization requested $85 million every two years to support global public health initiatives targeting drug-resistant infections. While the withdrawal of the United States from WHO introduces uncertainty regarding AMR initiatives, other regions have taken proactive steps:
- United Kingdom: Committed £210 million ($267.7 million) to monitor antibiotic-resistant bacteria across Asia and Africa, supporting the Fleming Fund global AMR surveillance program to detect dangerous pathogens before they spread to the UK.
- European Union: Launched a €253 million initiative in September 2025 to address AMR through coordinated research, monitoring, and public health strategies.
