An 18-year-old man exited an ambulance on a motorway and was Tasered by police before being struck by a vehicle, according to the police oversight body.
Logan Smith, from Weston-super-Mare, lost his life on the M5 in North Somerset late Sunday evening.
The Independent Office for Police Conduct reported that Smith was being transported to hospital when paramedics, concerned for his safety, stopped the ambulance on the hard shoulder of the northbound M5.
Police were called when Smith stepped onto the carriageway. Bodycam footage shows officers deploying a Taser, which caused him to fall, and he was subsequently hit by a passing car.
